Small business health plans

Our fully insured plans offer the coverage your employees need and the peace of mind that comes with fixed health care costs. It’s important to understand the difference between the types of plans and what benefits each offer.

• HMO: Coordinated care through a primary care physician
• EPO: Flexibility to seek services throughout Michigan
• PPO: Flexibility to seek services throughout Michigan as well as national coverage

A health savings account, or HSA, is a tax-exempt account used to pay for qualified medical expenses for the account holder and their dependents. A health reimbursement arrangement, or HRA, is an employer-funded account used to reimburse employees for out-of-pocket medical expenses. Use this chart to compare these options and choose what’s best for you and your employees.

Our narrow network plans give your employees access to nationally recognized doctors and hospitals from one health system or integrated network. This could save you money on your monthly premiums compared to our full HMO network plans. This is because doctors can work closely with others in the same network – easily viewing and sharing data, which cuts down on waste.

We have two choice plans:

• Henry Ford Choice - employees have access to the Henry Ford Choice network which includes doctors and facilities that are part of Henry Ford Health System. 

• Genesys Choice - employees have access to the Genesys Choice network which includes doctors and facilities that are part of the Genesys Physician Hospital Organization as well as health care facilities affiliated with Genesys Regional Medical Center. 

With our fully insured HMO plans, your business must be located within one of these Michigan counties or within the counties highlighted in orange in the map below.

In Michigan, members can seek in-network care through the HMO network of providers within the counties highlighted in orange.

Employees with a HAP HMO small group plan can seek care through our HMO network in Michigan. Learn more about HAP's network.

With our fully insured PPO plans, your business must be located within one of these Michigan counties or within the counties highlighted in orange in the map below.

Small group PPO sales and network coverage area:

Employees with a HAP PPO small group plan can seek care through our statewide PPO network in Michigan and Northwest Ohio* or use the Aetna Signature Administrators® Network when outside of that network.

With our fully insured EPO plans, your business must be located within one of these Michigan counties or within the counties highlighted in orange in the map below.

Small group EPO sales and network coverage area:

Employees with a HAP EPO small group plan can seek care through our statewide PPO network in Michigan and Northwest Ohio or use the Aetna Signature Administrators® Network if they permanently reside outside of Michigan.

Henry Ford Choice plan

With our Henry Ford Choice plan small businesses must be located in in Macomb, Oakland and Wayne counties. The Henry Ford Choice network includes doctors and other health care providers within the Henry Ford system.

Genesys Choice plan

With our Genesys Choice plan small businesses must be located in in Genesee county. The Genesys Choice network includes doctors and other health care providers within the Genesys Physician Hospital Organization. Facilities in this network include Genesys Regional Medical Center and affiliated urgent care centers.
