Contract information

Both Health Alliance Plan of Michigan and Alliance Health and Life Insurance Co. have a contract with the Centers for Medicare & Medicaid Services (CMS), the government agency that runs Medicare. These contracts can be renewed each year if both parties agree.

Disenrollment

Disenrollment means leaving HAP and no longer being a HAP member. If you decide you want to leave HAP's coverage, there are several ways you can do it.

New Medicare Members

Whether you are new to HAP Medicare or recently changed plans, register at hap.org to manage your account.

New EGWP Members

If you are a new EGWP member, register at hap.org after you receive your HAP ID card. You'll create a login to manage your personalized account.

New Medicare Supplement Members

Health Alliance Plan (HAP) has HMO, HMO-POS, PPO plans with Medicare contracts. HAP Medicare Complete Duals (HMO D-SNP) and HAP Medicare Complete Assist (PPO D-SNP) are Medicare health plans with Medicare contracts and a contract with the Michigan Medicaid Program. Enrollment depends on contract renewals.
