You have educational resources at your disposal to help you on your diabetes management journey. There are Henry Ford Health diabetes care experts who are ready to provide you with additional training and knowledge on living better. Also, if you need extra support, you can request a HAP case manager to assist you. Best of all, there is no cost to you.
Diabetic Supply Information (Pharmacy Transition)
HAP covers the following formulary diabetic testing supplies at the pharmacy. Some may require prior authorization or have step therapy. Continuous Glucose Monitors (CGM’s) coverage requires insulin use. Insulin must be in their claim’s history in the last 120 days. The items below will be part of the transition fill process, which allows one 30-day fill in the first 90-days of enrollment with HAP. A letter will be sent to you and your physician on the next steps for future coverage after the transition fill.
