HAP Empowered has you covered for COVID-19 tests
To get free tests at the pharmacy, you must:
Step 1: Have a prescription from your doctor. Your Medicaid pharmacist, primary care physician (PCP) or nurse practitioner can do this.
Step 2: Make sure it is one of the tests covered by the State of Michigan. Check out the full list of covered COVID-19 tests.
Once you have completed Step 1 and 2, your pharmacy will submit the bill for the tests to HAP Empowered to be paid.
Please note: There is a limit of 1 test per day. Your pharmacy can bill a box of 2 tests as a 2-day supply.
Get reimbursed for tests you already paid for at a store or online
If you already bought your COVID-19 tests at a store or online and need to be reimbursed, you must:
Step 1: Make sure it is one of the tests covered by the State of Michigan. Check out the full list of covered COVID-19 tests.
Step 2: Fill out the form below. Click on the form for your plan:
HAP Empowered MI Health Link (Medicare – Medicaid Plan)
HAP Empowered Dual Special Needs Plan (D-SNP)
Please note: HAP Empowered will pay you back for a max of $12 per test, 8 tests per calendar month.
Step 3: Mail the completed form with your test receipt or proof of purchase. Do not staple your receipt or proof of purchase document(s) to the reimbursement form. You can mail these documents to:

HAP Empowered also covers oral and injectable drugs to treat COVID-19, if needed.
The best way to stay healthy from COVID-19 is the vaccine. The COVID-19 vaccine is covered at no charge to HAP Empowered members.

HAP Empowered Duals (HMO SNP) is a Medicare health plan with a Medicare contract and a contract with Michigan Medicaid Program. Enrollment depends on contract renewal.
