This online health assessment helps you prioritize health changes. Your responses result in a unique online plan with strategies designed by healthcare professionals to help you make health choices, overcome pitfalls and achieve your goals.
Diabetes Care
HAP Diabetes Care is our personalized diabetes management approach offering a variety of ways to help you stay healthy by supporting your self-care plan for type 1 or type 2 diabetes.
Members work with their own provider(s) and clinical team to create a customized self-care plan for managing their diabetes. We are part of that coordination team.
Diabetes Care offers clinically-based support for you to:
- Find a physician to create a self-care plan to help monitor your critical health measures such as A1C, blood pressure, and cholesterol;
- Discuss appropriate diabetes management technology (a CGM or a glucometer) and low-cost supplies through our pharmacy partners;
Provide educational intervention resources to help manage your diabetes.
