HMO Plan Basics
When enrolled in HAP's HMO plan, you can choose your doctor from thousands of highly qualified primary care physicians (PCPs). HAP has one of the largest Michigan-based HMO networks,* so changes are, your doctor and hospital are already part of our established network of providers.

Managing Your Health

HAP offers the SilverSneakers® program for Medicare members and the Active&Fit Direct™ program for non-Medicare members, providing access to numerous fitness locations, virtual classes, and wellness coaching.
Hearing and Vision Benefits
HAP partners with NationsHearing to offer hearing aid benefits for Medicare-eligible members, including annual hearing aids, full coverage for initial tests, and adjustments. Costs vary by technology level, with Medicare members receiving significant discounts.
HAP partnership with EyeMed provides our member with a nation-wide network of doctors for their routine care needs.
Health Alliance Plan (HAP) has HMO, HMO-POS, PPO plans with Medicare contracts. HAP Medicare Complete Duals (HMO D-SNP) and HAP Medicare Complete Assist (PPO D-SNP) are Medicare health plans with Medicare contracts and a contract with the Michigan Medicaid Program. Enrollment depends on contract renewals.
