C-SNP and D-SNP Model of Care Training 

The Centers for Medicare & Medicaid Services requires contracted and noncontracted providers, who may care for HAP Medicare Complete Duals (HMO D-SNP), HAP Medicare Complete Assist (PPO D-SNP) and HAP Medicare Diabetes and Heart (HMO C-SNP) plan members to complete our model of care training initially and annually.

To take the required training, Medicare contracted provider offices can either:

1. Log in to your provider account and click on the pop-up asking if your office is compliant with regulatory standards.
2. Take the training by following the link below and complete the attestation.

   • MOC Training Attestation

We will contact noncontracted providers who provided services to HAP Medicare Complete Duals (HMO D-SNP), HAP Medicare Complete Assist (PPO D-SNP), and HAP Medicare Diabetes and Heart (HMO C-SNP) members.
