Flex+ Rewards
Beginning January 2026, Healthy Living Rewards and flex benefits will be on one card, the HAP Flex+ Rewards prepaid Visa®. Your new card should have arrived in the mail in December.
Make sure to activate your card. You can do that, check your card balance and view all of the ways you can spend your funds on the HAP member portal.
If you were enrolled in the Healthy Living Rewards program for 2025, you may still receive rewards you earned in 2025 through April of 2026. A voucher from InComm Incentives will be mailed to you. You can redeem your rewards online or by mail for your choice of a gift card from national retailers.
Remember, while you’ll only have one card, your flex benefit is separate. It gives you a set amount to spend on certain items. Your 2025 prepaid benefits Mastercard will no longer work after December 31, 2025.

Health Alliance Plan (HAP) has HMO, HMO C-SNP, HMO-POS, and PPO plans with Medicare contracts. HAP Medicare Complete Duals (HMO D-SNP), HAP Medicare Complete Assist (PPO D-SNP), and HAP CareSourceTM MI Coordinated Health (HMO D-SNP) are Medicare health plans with a Medicare contract and a contract with the Michigan Medicaid Program that provides benefits of both programs to enrollees.
