HAP collaboration with Oak Street celebrates the increased access to holistic healthcare for adults on Medicare in Genesee County
CHICAGO - June 11, 2019 - Oak Street Health, a network of health centers providing an innovative model of value-based primary care for Medicare patients, has opened in Burton, its sixth center in Michigan. Through collaboration with Health Alliance Plan (HAP) and other payers, local residents will gain access to a more holistic healthcare option that delivers more personal, equitable and accountable healthcare for all. A public grand opening celebration will take place at 11 a.m. on Wednesday, June 12 to formally introduce the local community to Oak Street Health’s unique, proven model of care. HAP Chief Medical Officer Dr. Michael Genord will speak at the celebratory event.
The Burton center, located at 3535 Saginaw Rd., north of Bristol Road and just south of Connell Street in Burton, will provide local residents with a healthcare experience that goes far beyond the scope of the traditional doctor’s office experience. Patients have access to complimentary transportation to and from appointments, Medicare education classes to help them understand their insurance benefits, and supplemental services in addition to primary care, such as podiatry and behavioral health, to address their healthcare needs in one convenient location.

For adults aged 65 and older, social activity is a proven, crucial component to overall wellness. Yet, as chronically ill adults age, isolation and depression is common. To address this, the Burton center provides social activities and classes for patients and community members so they can stay active and connected with peers.
This level of care doesn’t end when a patient leaves the center. Oak Street Health offers a 24/7 patient support phone line as well as complex care teams and home nursing visits for patients who need it the most. As a result, Oak Street Health has a 94 percent patient retention rate and an impressive 92 Net Promoter Score, which measures customer satisfaction.
Through its commitment to providing preventive services and improving accessibility of care, Oak Street Health has reduced hospital admissions by 44 percent, and reduced emergency room visits for its patients by 46 percent.

Oak Street Health will open a second location in Genesee County this year at 102 W. Pierson Road in Flint. Oak Street will accept multiple health plans, including HAP, at both locations. Michigan is already home to five other Oak Street Health centers, located in Southgate, Hazel Park, Inkster, and in the Jefferson Village and Rosedale Park neighborhoods of Detroit.
