HAP is committed to ensuring that your eye health is well taken care of. Through our partnership with EyeMed, we provide expanded access to a nationwide network of doctors for routine eye care needs. Here’s how you can make the most of your vision benefits as a HAP member in Michigan.

    • Each member can receive one annual eye exam at no cost through any in-network vision provider in the EyeMed provider network

    • Many providers offer extended weeknight and weekend hours, as well as online visits
    • Use the EyeMed provider directory to find and select providers offering online scheduling

Vision hardware discount clarification:

  • Hardware discount and large group rider benefit:

    • All HAP members in commercial groups receive a vision hardware discount.
    • Commercial large groups have the option to add a vision hardware rider.*
    • If the employer is a large group employer (more than 50 employees), it is advised that HAP members check their plan benefits to determine if their employer group plan has the hardware rider benefit. If it does, then once a member has utilized their maximum hardware benefit, the hardware discount can still be applied to additional hardware purchases.
    • Pediatric hardware is included in the Small Group and Individual Qualified Health Plans (QHP) base plan benefit for members up to age 19. 

    *Riders only apply to large group employer plans. Riders are not added to small group and individual QHP plans.

  • Know your vision hardware plan benefit and be prepared:

    • Check your specific vision plan benefit document in your member portal account under My Forms and Documents > My Plan Documents.
    • If your group plan includes a vision hardware rider, locate the document that states “Eyemed Vision Hardware Benefit Rider” to understand your allowed benefit amount.
    • The rider will state the allowed benefit amount toward vision hardware. If there is no rider document, then you will receive the automatic hardware discount.

    • Check your specific vision plan benefit document in your member portal account under My Forms and Documents > My Plan Documents.
    • Look for the document that states “Eyemed Vision Hardware Benefit Rider” to understand your allowed hardware benefit amount.
  3. Schedule an appointment:

    • Use the EyeMed provider directory to find providers offering online scheduling.
    • Remember to show your HAP ID card for all routine eye care visits.
