Plan basics

Special Enrollment Period

Typically, you can only enroll in a health plan during Open Enrollment Period (OEP). After OEP, you're locked into your health plan until the OEP in the next calendar year.  

If you've had a major life change or qualifying life event (like the birth of a child, marriage or losing your health coverage), you may be granted an exception. Your qualifying event allows you to enroll in a new health plan during what's called a special enrollment period or SEP.  

When applying for SEP, you will need to submit proof of the qualifying event with your application within 60 days.

Print the following application, and send a signed copy to the address on the form with any supporting documentation.

Existing HAP member? 

You can submit a member change form. You can call us to request one at (800) 422-4641 (TTY: 711) or log in to the member portal to retrieve the form.

Voluntarily canceling other health coverage or being terminated for not paying your premium is not considered loss of coverage. Neither is losing a plan that does not carry minimum essential coverage.
