By entering, you understand that you are agreeing to these Official Rules and to provide your information to Health Alliance Plan by Henry Ford Health to administer the contest (as defined herein). Void where prohibited by law.

This is a contest based on random drawing. These Official Rules apply only to the Win Your Wish List Contest. By entering, you will not be entered into any other contest, sweepstakes or promotion that Health Alliance Plan may be running simultaneously.
 

1. Contest Entry Period:

   The contest entry period begins at 12 p.m. EST, July 13, 2026, and ends at 11:59 p.m. EST, July 19, 2026. Eligible participants who follow Health Alliance Plan on social media and provide their wish list during this period will be entered to win. Names will be selected via random drawing.

   All winners will be announced publicly on social media on or about July 20, 2026. All winners will be notified by Meta private messenger on or about July 20, 2026, from a Health Alliance Plan social media representative.

   Health Alliance Plan will complete the winners’ Amazon wish lists provided in the comments section of the contest. Wish lists will be cleared up to $500 per winner.

   For the purposes of these Official Rules, “entrants” refers to eligible individuals who submit entries during the contest entry period in accordance with the terms outlined herein.
    

2. Eligibility:

   The contest is open only to teachers within the Lower Peninsula of Michigan who are 18 years of age or older.

   All entrants agree to be bound by the Official Rules, and the decisions of Health Alliance Plan shall be final and legally binding in all respects. All entrants hereby release Health Alliance Plan, its affiliated companies, subsidiaries, and all other businesses involved in this contest, as well as employees, officers, directors, shareholders, representatives, agents, and successors and assigns of the foregoing (collectively, the “Released Parties”), from all claims, liability, lawsuits, costs, expenses, fees, injuries (including death), losses, and damages relating to their participation, acceptance, and use or misuse of any prize (as defined below).

   Winners assume all liability for any injury or damage, including death, caused or claimed to be caused by participation in this contest or use or redemption of a prize. Winning a prize is contingent on fulfilling all requirements in these Official Rules.
    

3. How To Enter:

   During the Contest

   Entry Period you may enter by:

• Providing a link to your eligible classroom Amazon wish list on HAP’s Facebook contest post.

ENTRY NOTES:

Each entry:

(a) must not contain material that violates or infringes the rights of any other party, person or entity, including but not limited to copyright, trademark, privacy, publicity or any other intellectual property rights;

(b) must not in any way disparage Health Alliance Plan or any other person, entity or party;

(c) must not contain material that is inappropriate, indecent, obscene, hateful, tortious, defamatory, slanderous or libelous;

(d) must not contain material that promotes bigotry, racism, hatred or harm against any group or individual or promotes discrimination based on race, gender, religion, nationality, disability, sexual orientation or age;

(e) must not contain material that is unlawful or in violation of the laws or regulations of the United States or any jurisdiction where the submission is created;

(f) must not promote illegal drugs, firearms, unsafe activities, or any particular political agenda or message;

(g) must be consistent with the image and values of Health Alliance Plan and must satisfy the purpose of the contest.

Engaging directly or indirectly in any (apparent or suspected) artifice, cheating, fraud, spamming, phishing, online clubs (or similar methods), exchange sites, robotic, automatic, macro, programmed or similar participation methods, as determined in Health Alliance Plan’s sole and absolute discretion, will void all such entries and disqualify any entrant suspected of using or benefiting from such methods.

Without limiting the foregoing, an entry or entrant will be disqualified if Health Alliance Plan believes the entry is not in the spirit of the contest, is not compliant with these rules, or awarding the prize would have a detrimental impact on Health Alliance Plan, this contest, or any of Health Alliance Plan’s brands, products or services.

Entrants must not engage in phishing or spamming. Apparent or suspected phishing or spamming, sending of junk communications, or use of any unauthorized or automated system to participate is prohibited and, if discovered, will void the entry and disqualify the entrant at Health Alliance Plan’s sole discretion.

Once an entry is submitted, the entrant cannot access or revise it. In the event a dispute regarding the identity of the individual who submitted an entry cannot be resolved to Health Alliance Plan’s satisfaction, the affected entry will be deemed ineligible.

By submitting an entry, the entrant acknowledges that their name, image or likeness may be posted across any or all of Health Alliance Plan’s and other Released Parties’ digital and social assets and other media, at Health Alliance Plan’s discretion. Submission grants Health Alliance Plan and its agents the right to publish, use, adapt, edit, publicly perform and/or modify such entry in any way, in any and all media, including for use in advertising and marketing, without limitation and without consideration to the entrant.

4. Winner Selection:

At the conclusion of the entry period, Health Alliance Plan will hold a random drawing to select winners. The winners will be announced on or about July 20, 2026, and notified via Meta private messenger by the official Health Alliance Plan Facebook page that will request each winner to fill out an information form that collects the winner’s name, the winner’s Amazon Wishlist, the School District/Building the winner works in, school address, and mailing address for the wish list items. The number of winners will be determined by the cost of the wish lists for the randomly selected winners.

If a selected winner is unable to accept a prize in its entirety for any reason, they will forfeit the prize and it will be awarded to alternate winners.

By accepting a prize, winners irrevocably grant Health Alliance Plan the right and permission (except where prohibited by law) to use their name, prize won, likeness, photograph, voice, state of residence and biographical information in any media throughout Michigan in perpetuity for purposes of advertising, online posting and publicity in connection with the contest without additional compensation.
 

5. Winner Notification:

Winners will be notified by Health Alliance Plan via Meta private messenger with a message that will request each winner to fill out an information form that collects the winner’s name, the winner’s Amazon Wishlist, the School District/Building the winner works in, school address, and mailing address for the wish list items. Failure to respond to the notification or noncompliance with these Official Rules may result in disqualification, and at Health Alliance Plan’s sole discretion, alternate winners may be selected by random drawing.

6. Prize:

Subject to entrant’s compliance with these Official Rules and while supplies last, each winner will receive up to $500 toward their school supplies wish list. Wish list purchases will be completed by Health Alliance Plan.

Winners are responsible for all applicable federal, state and local taxes and any related expenses or fees.

To receive the prize, winners must coordinate with the Health Alliance Plan social media marketing team for delivery of a Health Alliance Plan swag bag to their school of employment. Winners must also create and share at least one Facebook post showing their use of the classroom supplies and Health Alliance Plan swag. Each post must:

(a) mention @HealthAlliancePlan;

(b) comply with Meta’s Community Guidelines, Terms of Use, Privacy Policy and all incorporated terms;

(c) contain content that is original, respectful and suitable for Health Alliance Plan’s audience;

(d) reflect the winner’s own honest opinions, beliefs and experiences;

(e) comply with all applicable laws and FTC guidelines on endorsements;

(f) refrain from offensive language, derogatory remarks, inappropriate imagery or objectionable content;

(g) refrain from making false or factually incorrect statements;

(h) refrain from violating the intellectual property rights of others;

(i) refrain from including personally identifiable information about anyone other than the winner; and

(j) refrain from using the name, image or likeness of anyone other than the winner.

IN NO EVENT WILL THE RELEASED PARTIES BE RESPONSIBLE OR LIABLE FOR ANY DAMAGES OR LOSSES OF ANY KIND, INCLUDING INDIRECT, INCIDENTAL, CONSEQUENTIAL OR PUNITIVE DAMAGES ARISING OUT OF ENTRANT’S PARTICIPATION IN THIS CONTEST OR FROM AN ENTRANT’S DOWNLOADING FROM AND/OR PRINTING MATERIAL DOWNLOADED FROM ANY WEBSITE ASSOCIATED WITH THIS CONTEST.
 

7. Conditions of Participation:

By entering the contest, each entrant agrees:

(1) to be bound by these official rules and by all applicable laws and the decisions of Health Alliance Plan, which shall be binding and final;

(2) to waive any rights to claim ambiguity with respect to these official rules;

(3) to waive all of his or her rights to bring any claim, action or proceeding against any contest entities or released parties in connection with the contest; and

(4) to forever and irrevocably agree to release, defend, indemnify and hold harmless each of the contest entities and released parties, and the social media platforms used to facilitate entry, from and against any and all alleged or actual claims, lawsuits, judgments, causes of action, proceedings, demands, fines, penalties, liability, costs and expenses (including, without limitation, reasonable outside attorneys’ fees) for any injuries, losses or damages of any kind to persons, including death, or property resulting in whole or in part, directly or indirectly, from:

(a) the contest, including but not limited to any contest-related activity or element thereof, and the entrant’s entry, participation or inability to participate in the contest or any component thereof;

(b) the violation of any third-party privacy, personal, publicity, proprietary or other rights;

(c) typographical or printing errors in these official rules or any contest materials and/or announcements;

(d) acceptance, attendance at, receipt, travel related to, participation in, delivery of, possession, defects in, use, non-use, misuse, inability to use, loss, damage, destruction, negligence or willful misconduct in connection with the use or misuse of a prize (or any component thereof);

(e) any change in the prizing (or any components thereof) due to unavailability or for reasons beyond Health Alliance Plan’s control, including but not limited to acts of God, any action, regulation, order or request by any governmental or quasi-governmental entity (whether or not such action, regulation, order or request proves to be invalid), equipment failure, threatened or actual terrorist acts, earthquake, war, fire, flood, explosion, unusually severe weather, hurricane, embargo, labor dispute or strike (whether legal or illegal), labor or material shortage, transportation interruption of any kind, work slowdown, civil disturbance, insurrection, riot, event cancellation or delays, or any other cause beyond any of the released parties’ control, or as otherwise permitted in these official rules;

(f) any interruptions in or postponement, cancellation or modification of the contest;

(g) human error;

(h) incorrect or inaccurate transcription, receipt or transmission, or any failure with respect thereto of any part of the entry (including, without limitation, the information or any parts thereof);

(i) any technical malfunctions or unavailability of the website, social media platform, telephone network, computer system, computer online system, computer timing and/or dating mechanism, computer equipment, software or internet service provider, social media platform or mail service used by any of the released parties or by an entrant;

(j) interruption or inability to access the contest, the website, the social media platform or any other contest-related web pages or platforms, or any online service via the internet due to hardware or software compatibility problems;

(k) any damage to entrant’s (or any third person’s) computer and/or its contents related to or resulting from any part of the contest;

(l) any lost or delayed data transmissions, omissions, interruptions, defects or any other errors or malfunctions;

(m) any late, lost, stolen, mutilated, misdirected, illegible, delayed, garbled, corrupted, destroyed, incomplete, undeliverable or damaged entries;

(n) any wrongful, negligent or unauthorized act or omission on the part of any of the released parties;

(o) lost, late, stolen, misdirected, damaged or destroyed prizing (or any element thereof);

(p) the collection, use and/or sharing of entrants’ personally identifiable information by Health Alliance Plan or its designees; or

(q) the negligence or willful misconduct by entrant.

You further agree that none of the contest entities will be liable to you under tort, contract, strict liability, negligence or any other legal or equitable theory for any loss or damages of any kind (including, without limitation, any direct, indirect, economic, exemplary, special, punitive, incidental or consequential losses or damages); that damages, if any, caused to you are not irreparable or sufficient to entitle you to an injunction preventing any exploitation of any website, service, property, product or other content owned or controlled by Health Alliance Plan; and that you will have no rights to enjoin or restrain the development, production, distribution, advertising, exhibition or exploitation of any website, property, product, service or other content owned or controlled by Health Alliance Plan.

You agree that Health Alliance Plan is not responsible for the actions, content, information or data of third parties, and you release the released parties from any claims and damages, known and unknown, arising out of or in any way connected with any claim you have against such third parties.

You also agree to the use of your name, voice, performance, photograph, caption, entry and/or likeness for programming, advertising, publicity and promotional purposes in any and all media, now or hereafter known, worldwide and on the internet, in perpetuity, by Health Alliance Plan and its designees, without compensation (unless prohibited by law) or additional consents from entrant or any third party, and without prior notice, approval or inspection, and to execute specific consent to such use.

Any failure by Health Alliance Plan to enforce any of these official rules will not constitute a waiver of such rules. If there is a conflict between any term of the official rules and any marketing or entry materials used in connection with the contest, the terms of the official rules will govern.

You represent and warrant that you have all necessary authority to (a) agree to the terms and conditions of these official rules and (b) perform fully any obligations you have under these terms and conditions.
