How does HAP Health Living Rewards Program work?

Simply put, we reward you for getting the tests you need. 

1. Get any of the tests or services outlined in the eligible services table. You may already be planning to get many of these tests.

2. You'll get rewarded automatically. Each quarter, we'll track your rewards by looking at the claims your HAP doctor sends us. Then we'll mail you a voucher for the value of the rewards you earned.

3. Redeem your rewards for your choice of a gift card from national retailers.

To be eligible, you must have a HAP Medicare Advantage plan. The tests or services (see eligibility chart) must be performed on the person who has registered, but all eligible members of a household can participate. The service or test must be completed between Jan. 1 and Dec. 31, 2020 to qualify for a reward. We'll let you know which test you're eligible for.

Qualifying services

See our list of qualifying services and their rewards. You may only receive one reward per type of test each year. For example, if you are a member with diabetes and get your HbA1c test more than once in the year, only one of the tests will qualify for a reward. You can, however, receive multiple rewards for different services received between Jan. 1 and Dec. 31, 2020. For example, based on your eligibility, if you received an HbA1c test and a colonoscopy between Jan. 1 and Dec. 31, 2020, both of those services would qualify.

How we track rewards

Each quarter, we’ll track your rewards automatically by looking at the claims your HAP doctor sends in to us.

  • If you have a health status discussion with your doctor, you can complete this form to let us know. 
  • For members with diabetes who had a retinal eye exam (retinopathy screening) and didn't use your HAP coverage, you may still be eligible for the incentive. Simply send us the information using this online form.

How we send your reward

InComm InCentives will mail you a voucher for the value of the rewards you earned, which is redeemable for a gift reward from national retailers. You can redeem your reward online or by mail. Rewards are sent quarterly, so you might not get it right away.
