Your Annual Wellness Visit is not a regular physical exam. It's a one-on-one, preventive check-in with your doctor to:
- Review your medical and family history
- Update prescriptions and vitamins
- Schedule vaccinations and preventive screenings
- Discuss your mental health, sleep and well-being
- Bring up any health concerns or questions
How to schedule your Annual Wellness Visit and earn $40.
- Step 1: Call your doctor and schedule your Annual Wellness Visit—it’s included in your Medicare plan.
- Step 2: Attend your visit and talk through your health questions with your doctor.
- Step 3: After your visit, a $40 reward will be mailed to your home address that is on file with HAP.
