If we don’t hear from you by September 1, we will send you the document listed above electronically. However, if you’d like to receive this document via postal mail instead, you can make this request at no charge to you. Simply log in at hap.org with your health plan ID and password and change your preference in Paperless Options.

HAP Senior Plus (hmo), HAP Senior Plus (hmo-pos) and Alliance Medicare PPO are plans with Medicare contracts. Enrollment in the plans depends on contract renewals.
