Our Health Engagement program supports members who want to improve their health. We reward healthy lifestyle choices by lowering out-of-pocket costs.
We offer employers two plan designs within the Health Engagement program: Aspire and Achieve.
The Aspire program rewards members who commit to making lifestyle changes, such as weight management and quitting smoking.
The Achieve program is similar to the Aspire program. The main difference is Achieve members commit to meeting measurable outcomes in their efforts toward healthier lifestyles. These outcomes include lowering:
Living a healthy lifestyle means you’re less likely to need costly health care. We pass those savings on to members who successfully complete or work toward their goals under the Aspire or Achieve plan types.
Each plan type has two tiers: the Enhanced option and the Standard option. New members start in the Enhanced plan for their plan type and have lower out-of-pocket costs for themselves and their dependents for their first 90 days of coverage. If you work toward or complete your lifestyle goals during that time, you keep these lower costs.
If you don’t work toward or complete your goals during the qualification period, you and your dependents will move into the Standard plan. This tier has access to the same high-quality care and doctors as the Enhanced plan, but it has higher out-of-pocket costs.
The basic process is similar for both the Aspire and Achieve plan types. Your primary care doctor must complete the Member Qualification Form, or MQF, within 90 days of the coverage start date. Log in to your account to download an MQF for your doctor to fill out.
Aspire members must score at least an 80 out of a possible 100 points on the MQF, and Achieve members must score at least an 85. A spouse who is covered under plans with the Health Engagement program must also complete the MQF.

Our health maintenance organization, or HMO, plans are available to Michigan residents in 19 counties:
Our preferred provider organization, or PPO, plans cover members throughout Michigan, as well as nationally through Cigna. Our exclusive provider organization, or EPO, plans cover members throughout Michigan as well.
Our Henry Ford Choice network is available to residents of Macomb and Wayne counties, as well as parts of Oakland County. This plan rewards members who take advantage of Henry Ford’s network of doctors, specialists and hospitals. Learn more about the Henry Ford Choice network.
Our Genesys Choice Network is available to residents of Genesee County. This plan provides access to the Genesys Physician Hospital Organization network of doctors and specialists, as well as health care facilities affiliated with Genesys Regional Medical Center.
