HMO vs. PPO plans

HAP is here with HMO and PPO plans plus benefits beyond what you’ve come to expect. 

HMO plan highlights for Employees Hired after 5/1/2021: 

  • Lower monthly premiums 
  • Includes chiropractic services and hearing aid coverage
  • $500 individual in-network benefit
  • 20% coinsurance to $500
  • $3,400 individual out of pocket maximum 

HMO plan highlights for Employees Hired Prior to 5/2/2021:

  • Lower monthly premiums
  • Includes chiropractic services and hearing aid coverage
  • $500 individual in-network benefit
  • 20% coinsurance to $500
  • $3,400 individual out of pocket maximum

PPO plan highlights for Employees hired prior to 5/2/2021:  

  • PPO network is nationwide 
  • Includes hearing aid coverage
  • $1,500 individual in-network deductible 
  • 30% in-network coinsurance to $1,250
  • $6,000 Individual out of pocket maximum 

All plans include the same covered drugs.
