HMO plan highlights for Employees Hired after 5/1/2021:
- Lower monthly premiums
- Includes chiropractic services and hearing aid coverage
- $500 individual in-network benefit
- 20% coinsurance to $500
- $3,400 individual out of pocket maximum
HMO plan highlights for Employees Hired Prior to 5/2/2021:
- Lower monthly premiums
- Includes chiropractic services and hearing aid coverage
- $500 individual in-network benefit
- 20% coinsurance to $500
- $3,400 individual out of pocket maximum
PPO plan highlights for Employees hired prior to 5/2/2021:
- PPO network is nationwide
- Includes hearing aid coverage
- $1,500 individual in-network deductible
- 30% in-network coinsurance to $1,250
- $6,000 Individual out of pocket maximum
All plans include the same covered drugs.
