Every year, the Centers for Medicare and Medicaid Services works with research companies to send two different surveys to randomly selected HAP Medicare members. These surveys are used to better understand members’ experiences with HAP and the quality of care provided to Medicare beneficiaries.
The Centers for Medicare and Medicaid Services and the research firm SPH Analytics send the Medicare Satisfaction Survey to randomly selected HAP Senior Plus® members.
By sharing your views about your HAP health plan, you can help other people with Medicare become better informed when choosing a health plan. Feedback from this annual survey, along with other data and information, is used to award Star ratings to health plans.
Participation in the Medicare Satisfaction Survey is voluntary and private. No one at HAP will see your survey answers. Your feedback won’t be shared with anyone other than authorized representatives of CMS and SPH Analytics.
If you received the survey but haven’t had a chance to complete it, you may receive a follow-up call from SPH Analytics. You’ll have the opportunity to complete the survey over the phone and discuss any questions you may have.
If you have questions about the survey, call the SPH Analytics Customer Service Helpline toll-free at (866) 864-8918 between 9 a.m. and 8 p.m. ET Monday through Friday.
The Medicare Health Outcomes Survey was created by the Centers for Medicare and Medicaid Services to help monitor and improve the quality of care provided to Medicare beneficiaries. Research firm DataStat Inc. mails the survey to randomly selected members.
The survey asks about your health, feelings and ability to do daily activities over a period of time.
If you receive this survey in the mail, please complete it as soon as possible. If you have questions about the survey, call DataStat at (877) 455-9241 between 9 a.m. and 9 p.m. ET Monday through Friday.

HAP Senior Plus (HMO)/(HMO-POS)/(PPO) and HAP Primary Choice Medicare (HMO) are health plans with Medicare contracts. HAP Empowered Duals (HMO SNP) is a Medicare health plan with a Medicare contract and a contract with the Michigan Medicaid Program. Enrollment in the plans depends on contract renewals. HAP Senior Plus (PPO) is a product of Alliance Health and Life Insurance company, a wholly owned subsidiary of HAP.
