These partners work with HAP to ensure you get the care you need. You may receive communications from our partners.

AdhereHealth: AdhereHealth helps you stay on track with your prescribed medication. 

Amwell: HAP partners with Amwell for Telehealth services. With Telehealth, members can see a doctor online 24/7 from anywhere.

Aspire Health: Aspire specializes in providing an extra layer of support to patients facing a serious illness. This partner provides home-based palliative care to our members with chronic illness who are near the end of life. 

Assist America: Assist America provides 24/7 travel assistance to HAP members. HAP members can call on Assist America when they’re traveling 100 miles or more from home, for up to 90 consecutive days per trip.

Convey Health: Convey manages our over-the-counter benefit for Medicare Advantage members. 

Delta Dental: All HAP Medicare plans come with preventive dental administered by Delta Dental. It includes two oral exams and two preventive cleanings, and set of bitewing X-rays per calendar year.

EyeMed: Is our vision provider for individual HAP Medicare plans. EyeMed Vision Care, the fastest growing vision benefits company in the U.S., is dedicated to helping members achieve clear, healthy vision by giving them easy access to a diverse and expansive network of providers across the U.S. including independent eye doctors and national/regional retail setting providers like LensCrafters®, Pearle Vision® and Target Optical®. 

Henry Ford Virtual Care: Talk to a Henry Ford Health System doctor 24/7 virtually anywhere - from your phone, computer or tablet.

Home Access Health Corporation: Home Access helps prevent chronic diseases through early detection by in-home testing. They help members complete testing and screenings on their own, at home and deliver the results.

Livongo: Livongo combines advanced technology with 24/7 coaching to help make managing diabetes easier. Members with diabetes who opt into the program get a free smart glucose meter along with test strips and lancets shopped to them. The smart meter can be used to communicate with Livongo diabetes health care professional to make it easier to stay healthy. 

Matrix Medical: Matrix makes it easy and convenient for you to get a risk-assessment. A nurse practitioner will visit your home and complete your health assessment. 

Mom's Meals: HAP members may be eligible to receive 14 days of fresh, nutritious, ready-to-heat meals delivered to their home after discharge from the hospital from Mom's Meals. This benefit is available for members with one or more chronic conditions (diabetes, chronic heart failure, hypertension and COPD for Henry Ford Tiered Access members only). 

National Kidney Foundation of Michigan: HAP Medicare members with pre-diabetes have access to the National Kidney Foundation of Michigan's Diabetes Prevention Program, which helps people at risk learn how to make lifestyle changes and stick to them.

NationsHearing: NationsHearing is here to help HAP Medicare members manage their hearing loss including administering hearing benefits and hearing aids. 

naviHealth: naviHealth helps our members who are admitted to SNFs, LTACs, and Inpatient rehab.

peerfit MoveMembers who utilize HAP’s fitness benefit will have credits available to them each month that can be used towards a gym membership, an in-home FitKit, and/or virtual or in person classes at fitness studio or a community center. Each month members will have the option to determine how they want to use their credits and have the option to make monthly changes or keep their credits tied to one particular offering such as a gym membership.

Protocall: Protocall is available to our members who need behavioral health support after hours. The hours of operation are 5 p.m. to 8 a.m. Monday through Friday, all day Saturday, Sunday and on HAP observed holidays. 

Quest HealthConnect/MedXM: Quest HealthConnect provides in-home Bone Mineral density exams and other in-home screenings.

Specialty Medicare Management (SMM): SMM performs face-to-face and telephonic care management services for our members including annual health risk assessments.

WebMD Health Services: iStrive for Better Health gives members access to a personalized digital wellness manager, powered by WebMD health services. Members get free, personalized health and wellness tools and interactive programs – all designed to help identify and achieve individual health and wellness goals.
