Keep your health on track

It can be difficult to manage a chronic condition such as diabetes or asthma. That’s why many of our HAP Medicare plans offer Restore CareTrack, a comprehensive disease management program.

Restore CareTrack provides education and support to help you follow your treatment plan and keep your daily health goals on track.

Who’s eligible to join Restore CareTrack?

The Restore CareTrack program is designed to help members with chronic conditions such as:

  • Asthma
  • Chronic kidney disease
  • Chronic obstructive pulmonary disease
  • Coronary artery disease
  • Depression
  • Diabetes
  • Heart failure
  • Hypertension
  • Osteoporosis

How will Restore CareTrack help me manage my chronic condition?

Once you join the program, you’ll have access to the following resources:

  • Nurse health coach to consult with over the phone and help you stay on track with your health goals
  • Pharmacist to monitor your medications
  • Behavioral health specialist to help you note emotional triggers and other conditions that can impact your ability to follow your treatment plan
  • Web-based tools, information and resources such as iStrive
  • Preventive screening education
  • Reporting sent to your primary care physician outlining potential gaps in care

You can join Restore CareTrack, refer someone else or to talk with us about the program by phone.
