Member Registration Help

The Member Registration application provides access to Member applications immediately after registration.

Member Information

Item Description
ID Number This is the Member's ID number which can be found on the member's ID card
Last Name Member's Last Name
First Name Member's First Name
Last four digits of your Social Security Number: The last 4 digits of the member's Social Security Number
Date of Birth: Member's Date of Birth. The date of birth should be entered in the following format: MM/DD/YYYY. (i.e. March 9, 1960 should be entered in as 03/09/1960) The member must be 18 years of age or older to register,unless he/she is a Personal Alliance member. Personal Alliance members can register at any age.

Confirm E-mail Address: confirm the E-mail address listed

Create Password

Item Description
Password Member's password. The member should select a password that is alphanumeric and between 5 and 8 characters in length. The password is not case-sensitive.
Confirm Password Re-enter the password

Create Challenge Question

Item Description
Challenge Questions Member's challenge question. A challenge question is required should a member forget their password.
Answer Member's challenge answer. This is the answer to the challenge question. This should be something the member can easily remember. The challenge answer is not case-sensitive.

Select Type of User

The user should select if he/she is the actual member that is being registered or a parent/legal guardian of a Personal Alliance member who is under 18 years old.

Frequently Asked Questions

Q:  How old do I have to be to register for online member applications?
A.  Members must be 18 years old or older to register to use online services. However, Personal Alliance members can register at any age. In addition, a responsible parent or legal guardian of a Personal Alliance member under 18 years old may register to use the online member applications on their behalf.Once the Personal Alliance member turns 18 years old, however, access for the parent or legal guardian will then be deactivated and the Personal Alliance member will have to re-register to use the online member applications.
Q:  What do I do when my member record cannot be found while I am trying to register?
A.  Confirm that the information you have entered (ID Number, last 4 digits of your social security number, and date of birth in MM/DD/YYYY format) is correct. Also, you must currently be an active member in order to successfully register. If you continue to have this problem, contact us.

Q:  What do I do when the application indicates that I am entering incorrect information but I have verified that I have entered everything in correctly?

A: Although you are entering your information correctly, this information does not match what HAP has on file for you. Please contact us.
