IMPORTANT INFORMATION ABOUT OUR PLANS AND PROVIDERS Alliance Medicare PPO serves people with Medicare who live in Wayne, Oakland, Macomb, Genesee, Lapeer, Livingston, Monroe, St. Clair, and Washtenaw counties. You may select doctors and hospitals within our 9-county network, or choose to receive medical care from any Medicare-participating doctor, specialist, or hospital anywhere in the country. You do not need a referral. Alliance Medicare PPO covers your care, whether it is in- or out-of-network. With the exception of emergency or urgent care, it may cost more to receive care from non-plan or non-preferred providers.
HAP Senior Plus-Henry Ford serves people with Medicare who live in Wayne, Oakland, and Macomb counties and prefer to use the Henry Ford Health System. The Personal Care Physician (PCP) you choose coordinates all the services and care you need through the select hospitals, doctors, and specialists affiliated with this plan.
If the Personal Care Physician you select belongs to the Henry Ford Medical Group, you will also see specialists, when needed, within the Henry Ford Medical Group.
Instead you may select a PCP from those affiliated with Henry Ford Health System but not part of the Henry Ford Medical Group. This PCP will still coordinate your care, but you may see specialists from the entire HAP Senior Plus-Henry Ford network. Our HAP Senior Plus-Henry Ford Provider Directory has more detailed information. It identifies which Henry Ford Health System physicians participate in this plan. Except in emergency situations or for out-of-area urgent care and renal dialysis services, you need to use HAP Senior Plus-Henry Ford providers.
HAP Senior Plus-Expanded Network serves people with Medicare who live in Wayne, Oakland, Macomb, Genesee, Lapeer, Livingston, Monroe, St. Clair, and Washtenaw counties. Your Personal Care Physician (PCP) coordinates your care, but you may seek needed care from any HAP Senior Plus-contracted specialist, and you may use any HAP Senior Plus-contracted hospital. With the exception of emergency or urgent care, or out-of-area renal dialysis, it may cost more to receive care from non-plan or non-preferred providers.
