Wellness targets
The chart below identifies Member Wellness Target groups at three
distinct levels. Please refer to your personalized Member Qualification
Form to determine your specific level. It’s important to note that level
two and three targets must be met during your annual qualification
period.

Member agrees to keep

services current
If tobacco/nicotine
user, member agrees to
treatment plan to quit
tobacco/nicotine use
within 12 months
If BMI is ≥30, member
agrees to treatment plan
with a goal of 5% total
weight loss within 12
months
If cholesterol is not at
an acceptable LDL-C
level, member agrees to
physician’s treatment plan
If blood pressure is
≥140/90 (if diabetic
≥140/80), member agrees
to physician’s treatment
plan
If member’s blood sugar
is ≥8.0, member agrees to
physician’s treatment plan
If member’s alcohol use
is outside the wellness
targets, member agrees to
physician’s treatment plan
Level Two
Member is up to date or
has scheduled all age/
gender- and disease-
specific services
Member is non-tobacco/
nicotine user*
Member must meet BMI
target of <30 or show 5%
total weight loss
If cholesterol is not at
an acceptable LDL-C
level, member agrees to
physician’s treatment plan
Member must meet blood
pressure target <140/90;
if diabetic <140/80
If member’s blood sugar
