To continue to enjoy lower costs annually, you and your covered spouse simply need to complete the following steps within 90 days of enrollment. It couldn’t be easier!

•Meet with your HAP-contracted personal care physician (PCP) to fll out and submit a completed

Member Qualifcation Form ( MQF )*

• Complete the online iStrive SM HealthMedia SUCCEED® Health Risk Assessment (HRA)

•Meet specifc wellness targets or commit to participating in health improvement programs or treatment plans

From Enhanced to Standard:

Failure to return a completed Member Qualifcation Form with a score of 85 points or above and/or the

iStrive SM HealthMedia SUCCEED® Health Risk Assessment within 90 days of coverage will result in your benefts being transferred to the Standard plan with higher out-of-pocket costs. You will remain in the Standard plan through the end of the beneft period and for the frst 90 days of your next beneft period.

If you are unable to meet program requirements due to extenuating circumstances, please call Client Services toll-free at (888) 819-2540 . We’ll work with you to determine an alternate way to meet program requirements.

*Remember, results from lab work completed in the last nine months qualify, but your physician must complete and sign the form.
