Wellness Targets

The chart below identifes Member Wellness Target groups in three distinct levels. Please refer to your personalized Member Qualifcation Form to determine your specifc level. Level two and three targets must be met during the annual qualifcation period.

Lifestyle Behavior Level One Level Two Level Three

Preventive Services 20 Points

Member agrees to keep age/gender and disease-specifc preventive tests current

Must show that member is up to date with all age/gender and disease-specifc tests

Must show that member is up to date with all age/gender and disease-specifc tests

Tobacco Use 20 Points

If tobacco user, member agrees to treatment plan to quit tobacco use within 12 months

Member is non-Tobacco user* Member is non-Tobacco user*

Weight Management 15 Points

If BMI is ≥30, member agrees to treatment plan with a goal of 5% weight loss within 12 months

Member must meet BMI target of <30 or show 5% weight loss.

Blood Pressure 15 Points

If blood pressure is ≥140/90; if diabetic ≥130/80, member agrees to physician’s treatment plan

Member must meet blood pressure target <140/90; if diabetic <130/80

Cholesterol Control 15 Points

If cholesterol is not an acceptable LDL-C level, member agrees to physician’s treatment plan

Member must meet acceptable LDL-C guidelines

Blood Sugar Control 10 Points

If member’s blood sugar is ≥8.0, members agree to physician’s treatment plan

Member must meet normal or acceptable HbA1c <8.0

Alcohol Use 5 Points

If member’s alcohol use is outside the wellness targets, member agrees to physician’s treatment plan
