Steps You Need to Take in Subsequent Years
To remain in the Enhanced plan in subsequent years, you must follow your PCP’s treatment plan to reach the wellness target. If you are renewing in the Health Engagement program and your physician selected a treatment plan for the Tobacco Lifestyle Behavior, you must provide proof that you completed a Tobacco Cessation program. You may be asked to provide a receipt. Please include your HAP ID when submitting your receipt to HAP.
Acceptable Tobacco Cessation Therapies include:
• Smoking Cessation Counseling Services provided by your PCP • Henry Ford Health System Smoking Intervention Program (SIP) • Henry Ford Health System Tobacco-Free-For-Life (TFFL)
• Prescription and Over-the-Counter Smoking Cessation Drugs or Products, such as nicotine patches, gum, nasal spray or inhalers
• Electronic Counseling Services with HealthMedia BREATHE® Program
Failure to comply with a treatment plan or to provide proof that you completed a Tobacco Cessation Program will result in your not receiving the 20 points in the Tobacco Lifestyle Behavior and will result in your benefts being transferred to the Standard plan with higher out-of-pocket costs.
