Personal Action Toward Health (PATH) is a six-week workshop that helps you learn how to manage the challenges of health conditions such as:
- Heart problems
- Lung problems
- Chronic kidney problems
People with all health conditions are welcome to attend. The program is not focused on a specific disease. By working closely with your group you will gain the confidence to manage your health. You will also learn how to:
- Express your questions and concerns with your doctors and family members
- Develop action plans to turn long term goals into smaller steps
- Create a healthy diet and exercise plan
- Overcome stress
- Increase your energy level
- Become more independent
- Manage your medications
Our specially trained leaders guide you through two and a half hours a week for six weeks. Classes are limited to 16 members and are very interactive.
