Get engaged with your health

Your HAP HMO plan includes health engagement. This is an exclusive program which can help reduce out-of-pocket costs when you make a commitment toward reaching your health goals.

There are two HMO levels of benefits available to Ford salaried employees:

The Enhanced option can save you money on your out-of-pocket costs, but requires a commitment on your part. In the first 90 days, you’ll visit your doctor and get a deeper understanding of your health risks. This will help you make healthy lifestyle choices and set healthy goals. (HAP covers these benefits once every calendar year.)

The Standard option provides access to the same quality care, but with higher copays, coinsurance and deductibles. At the end of the 90-day period, if you and/or your covered spouse have not completed the three easy steps, you will automatically be placed in the Standard option. You and your spouse will stay in the Standard option for the next 12 months.

The Enhanced option

As a new member in the Aspire health engagement program, you and your covered spouse will be placed in the Enhanced option with lower out-of-pocket costs for the first 90 days of coverage.

To stay in the Enhanced option, both you and your covered spouse need to do the following three easy steps within 90 days of your contract effective date (qualification period):

  • Visit with your HAP-contracted personal care physician (PCP) to fill out and submit a completed Member Qualification Form (MQF)
    • It is your responsibility to ensure that HAP receives the form before the end of your qualification period
    • Lab work within the last nine months can be used to complete the form but your physician must still complete and sign the MQF
  • Meet specific wellness targets or commit to participating in health improvement programs or treatment plans
    • Wellness Targets are listed on the Member Qualification Form (MQF) and you must score at least 80 points
    • Wellness Target requirements for tobacco lifestyle behavior for your specific level are listed in the member guide you received in the mail
  • Complete the online health assessment

The Standard option

At the end of the 90 day qualification period, if the above steps are not completed, you and your family will be moved to the Standard option for the remainder of the benefit year and the first three months of your next benefit year. If you are a renewing member you will have the opportunity to qualify for the Enhanced option during the 90 day qualification period.

The Standard option offers the same access to quality care and physicians, but you will have higher out-of-pocket costs.

Comparing costs - one member's experience

Let's explore the difference in cost between the Standard and Enhanced options. Here is a hypothetical situation for "Pam", an individual member, to consider. Upon running her first half marathon, Pam injures her knee. After seeing her PCP, she is referred to a specialist to help get her back on the running track. The specialist recommends outpatient surgery to help her recover. The surgery will cost $2,000. The chart below is a breakdown of the costs between the Enhanced and Standard options.

The Member Qualification Form

The Member Qualification Form (MQF) is a document your personal care physician (PCP) must complete and submit every year. HAP must receive the form within 90 days after your coverage begins on 1/1 of the plan year. Submitting completed forms for you and your spouse by the due date satisfies one of the three easy steps to remain in the money-saving Enhanced option.
