What is a health assessment and where do I find it?

A health assessment is a questionnaire asking you about your health. There are 40 to 50 questions and it takes about 20 minutes to complete. Once you complete your health assessment, you'll receive your personal health score and summary report. The report will include recommendations on how to maintain or improve your health.

To access your health assessment, log in to hap.org. Under “Quick Links,” click on “Health Assessment.”
