What are the requirements for becoming a HAP participating practitioner?

  • Complete a physician information form (PIF).
  • Must have admitting privileges at a HAP-contracted hospital, or be covering for a HAP-contracted physician. If you are a primary care physician, a contracted physician or specialist can also admit on your behalf.
  • Specialize in an area of practice credentialed by HAP.
  • Have a valid email address.
  • Be a member of a HAP-contracted physician hospital organization.
  • Provide the following:
    • State of Michigan license
    • CMS certification number
    • General liability certificate ($100,000 per occurrence and $300,000 aggregate)
    • W-9 (version 2007 or later)
    • Accreditation certificate
