What are the requirements for becoming a HAP participating practitioner?
- Complete a physician information form (PIF).
- Must have admitting privileges at a HAP-contracted hospital, or be covering for a HAP-contracted physician. If you are a primary care physician, a contracted physician or specialist can also admit on your behalf.
- Specialize in an area of practice credentialed by HAP.
- Have a valid email address.
- Be a member of a HAP-contracted physician hospital organization.
- Provide the following:
- State of Michigan license
- CMS certification number
- General liability certificate ($100,000 per occurrence and $300,000 aggregate)
- W-9 (version 2007 or later)
- Accreditation certificate
