Does HAP provide in-home health assessments?

HAP partners with Matrix Medical to provide free, in-home health visits for our Medicare Advantage members. During the visit, a board-certified nurse practitioner, or NP, will come to your home. You'll get:

  • Answers to your health-related questions
  • A general wellness exam
  • Suggestions for important screenings or other tests
  • A full review of the medicine you take to help make sure you get the most out of it


After the visit, you will receive a Personal Health Summary detailing your health information in one complete document. We share a similar document with your doctor, making future doctor's visits go more smoothly. If you have any questions, please contact us.
