Receiving your rewards


Q. Who's eligible for the HAP Empower Your Health program?

A. To be eligible, you must be enrolled in the HAP Empowered MI Health Link plan. You must have been enrolled during the date of service or test to be rewarded for completing the service or test.


Q. What are the rewards?

A. Reward amounts can be found here. Rewards are given in the form of a InComm InCentives voucher.


Q. How are the rewards redeemed?

A. Members who qualify for rewards will receive a voucher for the value of the rewards you earned from InComm InCentives. It’ll be mailed to your home. The voucher is then redeemed for a gift card. There's a long list of merchants to choose from. Vouchers can be redeemed online (follow the directions in the voucher) or by mail. A form will be sent with the voucher that you can use to redeem by mail.


Q. How many rewards can I get?

A. You can get a reward for each type of qualifying service you get in 2021. But remember, rewards are only given for the services in the chart. And the services have to be completed between Jan. 1 and Dec. 31, 2021. See eligibility chart


Q. Can I get more than one reward for each service?

A. No. You may only receive one reward per type of test/service each year. For example, if you are a member with diabetes and get your A1c tested more than once in the year, only one of the tests will qualify for a reward. You can receive additional rewards for different qualifying services received between Jan. 1 and Dec. 31, 2021. For example, if you received an A1c test and a mammogram between Jan. 1 and Dec. 31, 2021, both of those services would qualify.


Q. When do services need to be completed?

A. All services need to be completed by Dec. 31, 2021.


Q. How does HAP Empowered know I had the service?

A. When you submit the form with your doctor’s signature, HAP Empowered will move forward with having your reward sent out to you for the services completed. If your doctor has not signed the form, we’ll confirm the service by looking for the bill (called a claim) that your doctor will send us.

If you completed a home testing kit: HAP Empowered receives weekly reports about what kits you may have completed. We’ll review for kits that qualify for a reward.

If you completed the free medication review: HAP Empowered's pharmacy team will send weekly reports on who completed a medication review and therefore qualify for a reward.


Q. What if I completed a service that's eligible for a reward before Jan. 1, 2021 and I'm not due for it again this year?

A. In order to receive a reward, services have to be completed between Jan. 1 and Dec. 31, 2021. Services completed before Jan. 1 will not apply toward this reward program.


Q. How do I schedule my Medication Review?

A. Please call our Pharmacy team at (313) 664-8615 to schedule an appointment. They’re available Monday through Friday, from 8 a.m. to 5 p.m. If you need to call before or after business hours, please leave a message. Someone will return your call.


Q. How long does a Medication Review typically last?

A. It depends on how many medications you have. On average, a medication review can last 30 minutes.


Q. I reviewed my medications with my doctor/local pharmacist. Would this count as a Medication Review and qualify for the incentive?

A. No. The review needs to be completed by a HAP clinical pharmacist.


Q. How do I get my reward?

A. Your reward will be sent to you by mail from our vendor, InComm InCentives. It will be in the form of a voucher. This voucher will give you the choice of getting your gift card electronically or have a hard copy mailed to you.

  • If you choose an electronic gift card: Follow the directions in the voucher on how to obtain it.
  • If you choose a hard copy: Mail back the completed voucher to InComm InCentives with your selection choice. 

Q. When getting the voucher from InComm InCentives and requesting my gift card, when can I expect to receive the gift card?

A. It’s pretty quick. If you go online to pick your gift card, it will ship the same day or next business day, or you can get it via email. If you mail in the form, it will take about two weeks to get to you.


Q. How are rewards paid out if multiple services have been completed? In one lump sum or multiple gift cards?

A. This depends on when HAP Empowered is notified of the services. If we’re notified that multiple services have been completed at once, InComm InCentives will send you a voucher for the lump sum. You can split up the amount if you want. But know that some of the option have a minimum amount for each gift card. For example, if you have a voucher for $30, you could get two $15 gift cards from two different merchants or the same merchant.


Q. Do I have to claim a reward by a certain date?

A. No. You don’t have to claim your reward from InComm InCentives by a certain timeframe.
