HAP Wins Awards for Outstanding Care Management

You may not think of health insurance as a creative industry. But it takes a lot of creative problem-solving to deliver great health care. At HAP, our doctors, nurses, case managers and health plan experts continually strive to find new and better ways to deliver care.

They figure out what our members need and why they may not get it. And then they develop programs that solve the problems.

In 2018, the Michigan Association of Health Plans recognized that work with two prestigious Pinnacle Awards.

The MAHP is the industry voice for 14 health plans that cover more than 2.5 million Michigan residents. They promote high-quality, affordable, accessible health care. The Pinnacle awards recognize health plans that improve care, lower costs and increase services.

Awards are granted by a panel that represents state government, health professionals, small business and the media. They choose the projects that most improve the health of their communities and the state.

We’re excited to report that we received the following awards:

Clinical Service Improvement

For helping families to manage the complex needs of premature babies.

In 2016, we partnered with ProgenyHealth to help families with frail newborns. They are an industry leader in case management. They help families with both basic and medical needs. 

In the first year of the program, 255 newborns were managed through HAP’s partnership with ProgenyHealth. Outcomes included:

  • Outperformed ProgenyHealth’s national average length of stay of 16.5 days to 1.7 days, resulting in 14.8 ALOS (average length of stay)

  • ER visits were below the national average of 2 to 1.2 visits

  • Readmission rates decreased by 60 percent

Chronic Disease Management

For helping women, aged 65 and over, manage osteoporosis.

A fracture can be a sign of osteoporosis. This program helps women who have had a fracture get tested. Our case managers educate members about bone health. And they make appointments for testing. Or they schedule testing at home or in rehab for patients who can’t get to their doctor. This personal outreach has increased the percentage of women who get tested from 11 to 46 percent.
