Worksite Flu Shots
Health Alliance Plan is preparing for a busy worksite flu shot season. To schedule a Flu Shot Clinic at your worksite, contact a HAP approved flu shot provider directly or contact your HAP Account Executive.

Eligibility for the Flu Shot:
All HAP Health Maintenance Organization (HMO) and Alliance Health and Life (AHL) members are eligible to receive flu shots from an approved flu shot provider at no charge. As with visits to any healthcare provider, members must show their HAP ID card when receiving the flu vaccine.
Scheduling an On-site Flu Clinic:
Contact one of the HAP approved providers directly or your HAP Account Executive. Flu Clinics at the worksite are generally scheduled from November 1 through January 31. Many of the providers will send you a contract or service agreement so that you have all the details of the program in writing. We recommend that you request this from the provider directly, prior to your clinic date.
Fees:
Approved providers bill the HAP Claims Department for covered HAP members who receive a flu shot at the worksite. Payment for your non-HAP employees must be arranged by you (the employer group) and the flu shot provider. HAP reimburses providers through our Claims Department based on a valid HAP member identification number and a procedure code for flu vaccine administration. Thus, we do not reimburse providers for fees that are not associated with a specific member receiving a flu shot. We expect the fee for flu shots to be in the range of $25 - $32 per dose.
Some providers have minimum participation numbers. Minimum fees and service fees are the responsibility of the employer group.
Community Flu Shot Sites:
The below organizations provide flu shots throughout the community. To find a site nearest you, see below:
- Maxim Health Systems (CVS, Costco, Meijer, Rite Aid, Target, Walgreens, etc.)
- Visiting Nurses Association of Southeast Michigan (Community Centers, VeeGees Pharmacy, etc.)
- Michigan Visiting Nurses Association, Ann Arbor, (800) 842-5504 (Community Centers, Senior Centers, Churches, etc.)
Flu shots are also available at the HAP member's Primary Care Physician office and at a variety of community sites. To schedule a flu shot with the HAP member's PCP, they should contact their PCP office directly.
