Drug Formulary Lists
To ensure that you receive quality medications, HAP uses a drug formulary. A formulary is a list of covered drugs and their respective copay tiers that represent the prescription therapies believed to be a necessary part of a quality treatment program. These medications are selected by HAP in consultation with a team of health care providers.
The formulary may vary based on your coverage. If you are unsure of your prescription drug benefits, please review your HAP or Alliance contract or contact Client Services.
Medicare-eligible members of HAP Senior Plus or Alliance Medicare PPO should review their Evidence of Coverage or contact Client Services for a copy of their formulary.
Commercial Formulary
Most HAP plans use this formulary, including some Medicare group coverage.
