HAP offers great PPO/EPO options through Alliance Health and Life. With the PPO option, you have the flexibility to seek care from providers within or outside of the preferred network without referrals. Using out-of-network may cause increased coinsurance and deductibles.

The EPO option offers the freedom of a PPO plan without the out-of-network benefits. The EPO is the perfect option for employers who want to offer a self-referral option without out-of-network coverage.

• Financing. This product can be offered on an experience-rated or self-funded basis.
• Utilization reviews. The ongoing review of network providers' utilization decisions helps contain health care costs.
• Freedom of choice. Choose the PPO option to seek services outside the preferred network and still receive benefits that will be reimbursed at a lower level than in-network benefits. Or choose an EPO plan with no out-of-network option that allows you to see providers in your designated provider network without a referral.
• Cost control. The benefit design can encourage participants to utilize the physician network and provider discounts, holding your costs in line.
