Referrals and Prior Authorization

You do not need a referral to see any provider, but some services may require prior authorization (approval by Alliance Medicare PPO in advance).

When you use plan providers, the plan provider is responsible for obtaining prior authorization.

For all mental health and chemical dependency treatment, call Coordinated Behavioral Health Management (CBHM) department PRIOR to treatment at 1-800-444-5755. We encourage you to precertify services to ensure the maximum benefit available. CBHM is available Monday through Friday from 8am to 5pm. Assistance is also available for emergencies 24 hours a day, 7 days a week at number above. TDD users should call (313) 664-8000.

Seeing Non-Plan Providers

When you use non-plan providers, Alliance Medicare PPO does not require you or the non-plan provider to obtain prior authorization. We will not deny coverage for services from non-plan providers just because prior authorization was not obtained. However, services must be medically necessary or covered under the plan. We may review the services you receive from a non-plan provider after the fact, to ensure that the services were medically necessary or covered under the plan. If we determine that the services are not covered benefits, or were not medically necessary, you may be responsible for all of the cost.

In addition, because some services may require prior authorization, we encourage you to pre-authorize those services. Services that require prior authorization are indicated in the Benefits Chart of the Evidence of Coverage (Coming Soon). You can pre-authorize services by calling us at the number on the back of your ID card.
