Important note
By law, with some plans, enrollment is limited to specific times of the year.

• The Annual Enrollment Period for Medicare Prescription Drug Plans and for Medicare Advantage plans (Alliance Medicare PPO or HAP Senior Plus) runs from November 15 through December 31.
• The Open Enrollment Period for Medicare Advantage plans (Alliance Medicare PPO or HAP Senior Plus) runs from January 1 through March 31. During this period you may change Medicare Advantage plans (e.g., from a PPO to an HMO), as long as you do not add or drop prescription drug coverage. 

Other events in your life may create a "special enrollment period" (SEP).  Examples are the loss of employer or union-paid coverage and moving out of your current plan's service area.  Contact a HAP Medicare representative to get more information.

Enroll online through our Web site Complete an application form online and submit it directly to us. You will receive a confirmation letter once your application is submitted to Medicare.   Print and mail an application Download an application form, print it, and mail it to us at the address shown on the form.   Enroll online If you want to join a plan that includes Medicare prescription drug coverage, you can enroll online at www.medicare.gov. You can print instructions for online enrollment here.   Call us You can enroll by calling a HAP Medicare representative toll-free at: (800) 868-3153 or TDD (800) 956-4325 Office Hours: Monday - Friday 8 a.m. to 8 p.m. Saturday 8 a.m. to noon. At all other times, you may access our Interactive Voice Recording system at the same number. Extended hours from November 15 - March 1: 7 days a week, including holidays, 8 a.m. to 8 p.m.
