As a member of HAP Senior Plus, you still have original Medicare, but you are getting your Medicare services as a member of HAP Senior Plus. This means you will get most or all of your health services from the doctors, hospitals and other health providers that are part of the HAP Senior Plus plan you select.

HAP Senior Plus with Medicare Prescription Coverage offers four plans.

• The HAP Senior Plus 3 County HMO (Plan ID 004 and Plan ID 009) is open to eligible beneficiaries who live in Wayne, Oakland or Macomb County. The Henry Ford Health System Network, including Wyandotte and Bi-County Hospitals, serves these plans. Members must receive most of their routine care from Henry Ford providers.

• The HAP Senior Plus 9 County HMO (Plan ID 007 and Plan ID 010) is open to eligible beneficiaries who live in Wayne, Oakland, Macomb, Genesee, Livingston, Lapeer, St. Clair, Monroe, and Washtenaw County. An extended network of providers including Henry Ford Health System and Providence Medical Centers, from whom you must receive most of your routine care, serves these plans.

All four plans provide coverage for all of the usual Medicare services that are covered for everyone with Medicare. They also provide Medicare Prescription Drug Coverage and even some additional services and supplies. Since HAP Senior Plus is an HMO, you must get most or all of your routine care from the provider network specific to your plan. These are called “plan providers”. You will have to choose one of our plan providers to be your Personal Care Physician (PCP). Your PCP will provide or arrange for most or all of your covered services. Care or services you get from non-plan providers, including those outside your selected network, will not be covered, except in limited situations such as emergencies.
