Each Medicare plan offers different benefits that are attractive to different people, depending on their personal circumstances and their medical needs and concerns. There are several factors you’ll want to consider as you decide what’s right for you.
Alliance Medicare Supplement plans

- With HAP Senior Plus HMO, you choose a Personal Care Physician (PCP) who works closely with you to monitor your health and coordinates all your care
- Alliance Medicare PPO gives you flexibility and the freedom to see any Medicare-participating doctor you choose, without a referral

Our Medicare contract is renewed annually. The availability of coverage beyond the
end of the contract year is not guaranteed. You must continue to pay your Medicare
Part B premium if not otherwise paid for under Medicaid or by another third party.
Important information
- HAP Senior Plus and Alliance Medicare PPO are health plans with a Medicare contract.
- HAP Senior Plus members must use providers within their Personal Care Physician’s network for all routine care. Coverage for specialist services requires a referral from your Personal Care Physician, with some exceptions. If you choose to go to a doctor outside our network for routine care, you must pay for the services yourself. Neither HAP nor Original Medicare will pay for these services.
- You’re eligible to join HAP Senior Plus if you are enrolled in Medicare Part A and Part B, and you reside within the plan’s service area. For HAP Senior Plus — Henry Ford Health System Network, the service area includes Wayne, Oakland, and Macomb counties. For HAP Senior Plus — Expanded Network, the service area includes Wayne, Oakland, Macomb, Genesee, Lapeer, Livingston, Monroe, St. Clair, and Washtenaw counties.
- PPO is a product of Alliance Health and Life Insurance Company, a wholly owned subsidiary of HAP. Alliance Medicare PPO members can choose between in-network and out-of-network providers. Services received for routine care from Medicare-participating providers outside of our network will generally cost more (i.e., have higher copay and co-insurance costs) except in emergency or urgent care situations.
- Alliance Medicare Rx is a prescription drug plan with a Medicare contract. You’re eligible to join Alliance Medicare Rx if you have Medicare Part A or Part B and you reside within the plan’s service area, which includes the entire state of Michigan.
