In the HMO option, you choose a personal care physician (PCP) from our network of providers to coordinate all routine medical care and arrange for specialty care. The PCP ensures that you receive the best care available, at the right time and in the right setting.

The HMO option is ideal for:

  • Groups that want powerful in-network coverage at a stable rate.
  • Employers seeking added value and worksite wellness programs.
  • Families with children who want low out-of-pocket costs for routine care.

Value Plus Options

HAP's Value Plus product offers the same benefits as our typical HMO with a few changes that encourage consumerism:

  • Annual out-of-pocket maximums.
  • A wide range of deductibles and coinsurance options, with lower premiums.

The ideal candidates for this product include:

  • Employers who want health care at a lower premium cost.
  • Employers focused on offering flexible options.
  • Employees seeking wellness and preventive care who are willing to share the risk.

ADVANTAGES OF OUR HMO:

Powerful in-network benefits such as:

  • Health maintenance. Plans are designed to encourage routine care, early detection and even prevention or management of illness. Routine, preventive care is covered with just a copay.
  • Maximum cost utilization control. For increased savings to your group, employees use their personally chosen PCP to oversee all of their care.
  • Coordinated care. Health care is coordinated at one source with the employee's PCP managing all medical care needs.
  • Emergency coverage worldwide. Emergency care is available worldwide with the HAP identification card.
  • Stable rates. This product is community-rated. All employees and employer groups are pooled together for rating purposes. As a result, relatively consistent rates can be expected from year to year.
  • Flexible cost sharing. A range of copayment selections and benefit designs are available.
  • HAP Advantage discounts. Members receive preferred rates on services that are not covered benefits, including Weight Watchers®, LASIK (Laser in-situ Keratomileusis) procedures and Globalfit Fitness Solutions.
  • Disease management. Programs and information are provided to help members with chronic diseases improve their health and quality of life. Some of these programs include HAP’s HealthTrack for asthma, smoking cessation, diabetes, depression and congestive heart failure.
