Did you know that you can create a provider directory based on your search criteria? 

Using our Provider Search and PCP Select, you can identify physicians within a specified mile radius, who speak the languages you seek and more.

How to Create a Personalized Directory
Get started on your own directory by following these simple steps:

  1. Visit the Provider Search and PCP Select function and search for providers that meet your criteria.
  2. On the the results page, click on Download to PC.
  3. Enter an email address where you would like the directory sent. A pdf format of your customized directory will be sent to the email address.
