All Other Plans: Polypharmacy
Polypharmacy is the unwanted duplication of drugs, and often results when patients go to multiple physicians or pharmacies. Typically, polypharmacy occurs when prescribed medications duplicate or interact with each other. However, it may also include:
-
dosages that are too high or too low;
-
medications incorrectly prescribed or filled; or
-
herbal medications interacting with physician-prescribed medications.
Polypharmacy can result in an adverse drug event, yet many such episodes can be prevented. If you're concerned that you may be at risk for polypharmacy, ask your physician or pharmacist for a complete medication review.
Below you will find information to help you understand polypharmacy, as well as a medication list to help you keep track of your medications.
