HAP has Facility & Medical Record Standards, which also encompass confidentiality, for all PCPs, specialists and behavioral health practitioners. Compliance with these standards is a contractual requirement and performance against the standards is part of the credentialing process. The standards incorporate National Committee for Quality Assurance (NCQA) and Office of Financial and Insurance Services (OFIS) site requirements for HMOs; they are updated frequently.
