HAP's In Home Care Program
About the Program HAP's In Home Care Program is for our members who have chronic illnesses and difficulty managing them alone. The program is offered at no additional cost to our members. HAP has partnered with Henry Ford Community Care Services, St. John Home Care and @Home Support to offer this program to all HAP members.
HAP's "In-Home Care" program differs from traditional home care in that it provides additional treatment options not available in routine home care visits.
This may be an option that you want to consider if you suffer from any of the following ailments:
- Respiratory problems
- Heart disease
- Cancer
- Debility (poor nutrition, weight loss, difficulty with walking, high risk of falls and difficulty with self care)
How to Join the Program If you are interested in the program, please speak with your personal care physician (PCP) or specialty care physician to begin the referral process. In addition to the referral, you must meet eligibility criteria to join the program. If you meet the criteria, you will be contacted by the agency to coordinate your care.

* During the annual enrollment period of November 15 through March 1 each year, Client Services Specialists are available seven days a week, including holidays, from 8 a.m. to 8 p.m.
