Osteoporosis
Osteoporosis, or porous bone, is a disease that thins and weakens the bones to the point that they become fragile and break easily. Osteoporosis increases the chance of breaking bones such as a hip, spine or wrist especially after a fall. It is most common in women over 50 years of age. Those at risk must talk with their doctor to ensure they are getting the proper nutrition, exercise, medications, and diagnostic tests.
About HAP’s HealthTrack Program for Osteoporosis
HAP's HealthTrack program is available to all HAP members ages 18 and older who are diagnosed with osteoporosis. The program supports your relationship with your physician by helping you stick to your prescribed treatment plans.
You’ll get a personal nurse to work one-on-one with you. Your personal nurse can help you
- Assess your readiness to make behavior changes
- Identify specific challenges
- Build self-confidence to manage your osteoporosis
- Set goals
- Provide information about osteoporosis
- Navigate the health care system
