Diabetes
Diabetes is a disease in which the body is unable to properly use and store glucose (a form of sugar). Glucose will then back up in the bloodstream, causing one's blood glucose (sometimes referred to as blood sugar) to rise too high.
There are several types of diabetes, Type 1, Type 2 and Gestational.
- Type 1 - In this type of diabetes, there is no insulin produced; causing glucose levels to rise in the blood stream. People with type 1 diabetes have to be treated with insulin to survive.
- Type 2 - In this type of diabetes, insulin may be produced but there is not enough insulin and the insulin does not work well.
- Gestational Diabetes - Gestational diabetes starts when your body is not able to make and use all the insulin it needs for pregnancy.
About HAP's HealthTrack Diabetes Program
HAP's HealthTrack Diabetes program is available to all HAP members ages 18 and older who are diagnosed with diabetes. The program supports your relationship with your physician by helping you stick to your prescribed treatment plans.
You'll get a personal nurse to work one-on-one with you. Your personal nurse can help you
- Assess your readiness to make behavior changes
- Identify specific challenges
- Build self-confidence to manage your diabetes
- Set goals
- Provide information about diabetes
- Navigate the health care system
