Chronic Obstructive Pulmonary Disease is often called COPD. COPD results in swelling of the air passages and destruction of the air sacs in your lungs. Emphysema and Chronic Bronchitis are the most common forms of COPD.
About HAP's HealthTrack COPD Program
HAP's HealthTrack COPD program is available to all HAP members ages 18 and older who are diagnosed with COPD. The program supports your relationship with your physician by helping you stick to your prescribed treatment plans.
