Chronic Kidney Disease (CKD)
Chronic kidney disease is the permanent loss of kidney function. It may be caused by physical injury or a disease that damages the kidneys, such as diabetes or high blood pressure. Early stages do not have any noticeable symptoms, and as it progresses, it can lead to kidney failure and require dialysis or kidney transplant to maintain life.
About HAP's HealthTrack Program for Chronic Kidney Disease
HAP's HealthTrack program for Chronic Kidney Disease is available to all HAP members ages 18 and older who are diagnosed with CKD. The program supports your relationship with your physician by helping you stick to your prescribed treatment plans.
You'll get a personal nurse to work one-on-one with you. Your personal nurse can help you
- Assess your readiness to make behavior changes
- Identify specific challenges
- Build self-confidence to manage your CKD
- Set goals
- Provide information about CKD
- Navigate the health care system
