Our award-winning program offers comprehensive services that are distinct from any other health plan.
Our interventions include:
- An Innovative Software-Based Registry that identifies HAP members in need of assistance.
- A Personal Nurse who works one on one with you.
- Pharmacists who monitor if you are taking more medications than needed, not taking the appropriate medications or if there needs to be dosage adjustments.
- Behavioral health specialists to help you identify emotional triggers and behavioral conditions that impact your ability to follow your physician's treatment plan.
- In-Home Telemedicine Monitoring Program that helps you and your nurse monitor your condition.
- A 6-week education program to teach you how to effectively manage your conditions. Sessions are held in convenient locations near your PCP's office throughout the year.
- A Report sent to your personal care physician, listing any gaps in care
- The Health Chronicle newsletter, offering health tips, seasonal reminders and health-conscious recipes.
HAP's HealthTrack program is available to all HAP members ages 18 and older who are diagnosed with one of the conditions of focus. A program is also available for children with asthma ages 5-17.
