About Our Program
HAP's HealthTrack is a comprehensive disease management program that supports your patient-physician relationship with the goal of helping you stick to your prescribed treatment plans.
HAP's HealthTrack has consolidated our disease management services into one comprehensive program to help our members better manage their chronic conditions. HAP's HealthTrack program covers a variety of medical conditions including:
HAP's HealthTrack program is available to all HAP members ages 18 and older who are diagnosed with one of the conditions of focus. A program is also available for children with Asthma ages 5-17.
HAP's HealthTrack program can help you understand your condition(s) and how to take steps toward better managing it. For more information, contact the Clinical Care Management Department at 1-800-288-2902, from 8:30 a.m. to 4:30 p.m., Monday through Friday. You can also email us at HAPsHealthTrack@hap.org. Our nurses work one-on-one with you to develop an action plan, set goals and track behavioral changes that can improve your health. For your convenience our nurse case managers are available until 8:00 p.m., Monday through Thursday. It's a true patient-centered approach to chronic disease management.
